Understanding hormonal changes

Hormones are chemical messengers that regulate virtually every function in your body, from metabolism and mood to sleep and skin health. The major hormonal shifts women experience during perimenopause and menopause affect estrogen, progesterone, testosterone, DHEA, cortisol, insulin, and thyroid hormones.

These changes don't happen all at once. Perimenopause can begin as early as your mid-30s, with symptoms intensifying through your 40s. Estrogen fluctuates wildly before eventually declining, while progesterone drops more steadily. This imbalance, often called “estrogen dominance,” drives many of the mood, weight, and sleep symptoms women experience.

Hormone replacement therapy (HRT) is the most effective treatment for the symptoms of menopause and has been shown to be safe for most women when started within 10 years of menopause. Modern HRT uses bioidentical hormones at physiological doses, a very different approach from the synthetic hormones of past decades.

Is HRT safe?

For most women under 60 or within 10 years of menopause, the benefits of HRT outweigh the risks. Major medical organizations including the North American Menopause Society (NAMS), the Endocrine Society, and the British Menopause Society support its use. Modern HRT uses bioidentical hormones and transdermal delivery methods that have a much better safety profile than the oral synthetic hormones studied in the early 2000s.

When should I consider HRT?

If menopause symptoms are affecting your quality of life, sleep, work, or relationships, HRT may be appropriate. The “window of opportunity” for starting HRT is within 10 years of menopause onset, when the cardiovascular and bone benefits are greatest. A clinician can help determine if HRT is right for your specific situation.
